Problem:

Defect code :  1. Error

XSH 6 (ft) Section 2.2.2 "The Name Space" states that
"The prefixes posix_, POSIX_ and _POSIX are reserved for use
by IEEE Std 1003.1-2001 and other POSIX standards. Implementations 
may add symbols to the headers shown in the
following table, provided the identifiers.... do not use
the reserved prefixes posix_, POSIX_ or _POSIX."

however, the last line(l642) of the following table on page 16/17
reserves the same prefixes for use in any header.

There is thus a contradiction between the two parts of the
standard.

Action:

Delete "POSIX_, _POSIX_, posix_" from the Prefix column
of the table on page 17 , line 42.
